Psychological Evaluations

A psychological evaluation involves a formal process to examine some aspect of a person’s psychological functioning. The most common goal of a psychological evaluation is to help establish a diagnosis for the emotional/psychological condition the individual is having and then make recommendations. Other possible goals of a psychological evaluation include making recommendations regarding emotional and behavioral approaches that can help with complex medical conditions, psychological factors that may be impacting a patient’s medical care and helping determine an individual’s degree of psychological disability. Psychological evaluations can also be useful in determining cognitive functioning and learning problems.   A psychological evaluation will typically involve a clinical interview with the psychologist (usually lasting approximately an hour) along with the completion of formal psychological testing. The testing may involve the administration of one or more standardized tests or batteries of tests, questionnaires, or surveys. How much time the testing takes can vary considerably depending upon the specific referral questions and purpose of the evaluation.  The psychologist will score and interpret the testing results and produce a written summary report that will either be given to the patient, or sent to relevant parties, such as a physician’s office, or clinic with the patient’s authorization.

Why do I need a psychological evaluation before I get a spinal cord stimulator?

Having a psychological evaluation prior to implantation of electrical medical devices has been a requirement of all insurance companies for over thirty years.   Spinal cord stimulators have been in use for about forty years and were developed based upon scientific research done over fifty years ago.   They are being used more often today due to vast improvements in technology and also the growing need find better methods to help patients with chronic pain without use of opioid pain medications.  Your referral for psychological evaluation does not indicate that your doctor thinks that you have psychological problems or that your pain is not real. It is based upon outcome studies that have found that the presence of unrecognized mental health issues is associated with heightened risk of poor outcome from the implanted devices.  These issues may include unrecognized conditions, unrealistic expectations, or poor comprehension of the impact of the device.   The evaluation process includes an interview with a psychologist and completion of necessary paperwork and assessment materials.  The psychologist will put together a summary report that is forwarded to your doctor in a few days.   The psychological interview also offers the opportunity for you to ask questions and clarify your understanding of the device and its impact on your life.   Many patients find the psychological interview helpful.
